This Driver Data Consent Form ("Consent Form") is provided in accordance with the US State Privacy Legislation, Montana CDPA (Montana CDPA is in force as of October 1, 2024) and other applicable data protection laws. This Consent Form explains how Company collects, uses, and shares your personal data in connection with the agreement between you (the "Driver") and Company.
A. Company collects and processes your personal data for the purpose of:
(i) Concluding and maintaining an agreement for transportation services with you.
(ii) Complying with legal and regulatory obligations.
(iii) Facilitating communications between Company and the Driver.
(iv) Managing and administering your account with Company.
(v) Ensuring the safety and security of the Company’s operations.

(i) Name, address, and contact information (phone number, email address, address).
(ii) Driver's license information.
(iii) Vehicle registration details.
(iv) Insurance information.

(vi) Location data while using Company's services, equipment.
(vii) Medical qualification requirements in accordance with FMCSA.
